We offer our sympathies for the loss of your loved one. To assist during this difficult time, Tel Aviv Sourasky Medical Center offers the following services:

 

Burial license

Issued on-site at the Medical Center Reception Office, Sourasky Building, Ground Floor, Wing C

Sunday to Thursday from 7:00 a.m. to 4:00 p.m.

Fridays and holiday eve hours: 7:00 a.m. to 12:00 p.m.

Funeral arrangements

To arrange burial and schedule the funeral date, visit the Israeli Jewish Burial Society (Chevra Kadisha) office in the hospital. Make sure to bring with the original burial license.

The Chevra Kadisha at Sheba Medical Center in Tel Hashomer operates every day from 8:30 a.m. to 6:45 p.m. and Saturday evenings beginning an hour and a half after the Sabbath is over.

Important note: In addition to scheduling the funeral time with the call center, the family needs to meet with a Chevra Kadisha representative at the Medical Center in order to register the deceased and provide the original burial license.

 

Obtaining a death certificate

To receive a death certificate of a loved one, you may fill out an application form at the Medical Center. The document will be mailed three weeks after the person has passed away.

If you did not filled out an application for a death certificate at the Medical Center, you may contact the Ministry of Interior three weeks after the person has died.
