​Emergency Room admission and discharge

Tel Aviv Sourasky Medical Center has three emergency rooms:

  • The General Emergency Room
  • Dana-Dwek Children's Hospital ER
  • Lis Maternity and Women's Hospital Admission Unit

 

Below is information about admission, discharge, and payment for medical services.

Emergency admission

All patients should register in ER admissions upon arrival and present personal identification and a medical referral form. Patients can present a financial guarantee form (from their HMO, insurance company, the IDF, etc.) or pay at the end of treatment (according to the standard rates set by the Ministry of Health).

When patients receiving care without presenting an insurer payment guarantee in advance, their insurers may not necessarily be obligated to bear the cost of service or part thereof.

 

An insured HMO patient who visits the Emergency Room for one of the conditions listed below is exempt from payment. In these circumstances, the hospital will work directly with the HMO to secure payment.

1.       The insured patient visits the ER with a medical letter and/or HMO guarantee form (Form 17), but was not hospitalized.

2.       An insured patient who was hospitalized even if he or she did not have a medical letter and/or Form 17.

3.       A person who sustained a work injury who brings a work-injury payment authorization form (National Insurance Institute of Israel Form 250) from his/her employer.

4.       Car accident victims – up to 48 hours after the accident.

5.       A student who was injured at school or during a school trip who has a school approval document to visit the ER.

6.       Insured patients visiting the Emergency Room the following cases:

  • New bone break
  • Severe dislocation of the shoulder or a joint
  • An injury requiring stitches or other wound closure technique
  • Inhalation of a foreign object in the airway
  • Penetration of a foreign object into the eye
  • Cancer treatment
  • Hemophilia treatment
  • Cystic fibrosis treatment
  • A woman with labor contractions
  • A person taken to hospital by Magen David Adom ambulance, or from the street, or another public place following a sudden unexpected event
  • A baby under age two who spikes a sudden high fever (over 101.3°F or 38.5°C).
  • Dialysis patients
  • Sexual assault victims
  • Domestic violence victims

 

Discharge from the Emergency Room

Once discharged from the ER, the patient should go to the Admissions Office to complete administrative and financial arrangements and to receive a medical summary with the prescribed the course of treatment and instructions.

If the patient is hospitalized following their ER visit, information on administrative arrangements is available from the admissions/discharge reception desk.

A patient whose payment for emergency room treatment is not settled via one of the options listed above is required to provide the ER with a financial guarantee from his/her insurer or pay for treatment within two weeks of discharge.

 

Submitting a financial guarantee

The guarantee can be sent via mail or fax based on the ER where the patient was treated (see contact details below).

 

Cash payment

Payment can be made by credit card (also can be called in by telephone), cash, Israeli check, or foreign currency.

  • Dana-Dwek Children's Hospital Admissions Office
  • Lis Maternity Hospital Admissions Office

 

Below is information about admission, discharge and payment procedures for medical services.

What to bring

  • Hospital unit hospitalization order
  • HMO, IDF, or insurance guarantee, or payment for hospitalization
  • ID card

 

What not to bring

The Medical Center recommends not bringing items of value, such as jewelry, cell phones, laptop computers/tablets, personal items, and documents that are not necessary for the medical stay. In special cases, and when there is no other option, items of value will be stored in the Admissions Office safe.

 

Hospital admission from the Emergency Room

When the ER decides that a patient needs to be admitted, the Admissions Office will work directly with the HMO or IDF for payment arrangements. Patients without health insurance should arrange payment with the Admissions Office.

Discharge

Administrative discharge is usually performed by the hospital unit secretary, except in cases where the patient is directed to be discharged by the Admissions Office. The discharge summary and medical instructions will be provided by the hospital department staff.

Maternity discharge: Lis Maternity and Women's Hospital

Below is information on mother and baby discharge, baby registry, operating hours, phone numbers, and location.

 

Visit the Lis Reception Office

Present an ID card and referral form (if patient has one). The office will open a medical record and the patient will be directed to Maternity Admissions.

Medical treatment at Lis needs to be paid for with an HMO financial guarantee (Form 17) or through private payment.

If the mother was hospitalized and/or remained in the hospital to deliver, in most cases, the payment is arranged directly between the hospital and the HMO/National Insurance Institute of Israel.

 

Information on the mother's discharge and the baby registry

The baby registration process and the mother's discharge are done at the Registration Office, located across from the Pediatrics Unit.

Hours: Sunday to Thursday from 10:00 a.m. to 1:00 p.m.; Fridays, Saturdays, and holidays from 9:00 a.m. to 12:00 pm

The registration process includes:

  • Getting a newborn birth notification from the Ministry of Interior
  • Receiving a birth subsidy from the National Insurance Institute of Israel
  • Getting a birth certificate from the hospital

 

Documents required:

  • For a married woman: A current identity card (including the attachment) of both partners. Both parents must be present for registration and declaration of paternity
  • For a divorced woman: A current identity card (including the attachment) and divorce certificate
  • For a widowed woman: A current identity card (including the attachment) and the husband's death certificate
  • For a single woman: A current identity card (including the attachment)

To obtain the birth grant from the National Insurance Institute, when registering a first-born child, the mother should supply her bank account details or the couple's joint bank account information.

 

Tourists/foreign workers: Present a passport and marriage certificate (if married). Workers whose employers pay national insurance contributions for them are entitled to receive both a one-time birth grant from the National Insurance Institute and payment of hospital delivery costs – after all requisite forms are submitted.
